By filling out the form below, you attest that that in good faith, you have watched the educational video(s) or tutorial(s) from our channel in full.  Your request for credit will be sent to our Education Chair, who will file the proper paperwork for CEU credit. This process can take up to 4-5 weeks to appear in your records. If for any reason we cannot issue credit or there is an issue, we will reach out to you.
